Conceptualize why you are using a form in your teaching?
These are some of the reasons why teacher may choose to use forms.
Are you trying to gather data prior to teaching a unit to check for prior knowledge with your students?
Are you trying to gather student/parent data from the parents?
Are you trying to use a form as an online test?
You are going to create 2 forms that are relative to your teaching. When each form is completed you will post the link in a common place for the class to review. Follow posting the links (no later than 5/11) each participant will complete 4 forms (no later than 5/14) and indicate completion on the g-doc. Create 2 seperate lists of questions (10) with a mix of open ended and multiple choice responses in an office document, this will ensure spelling and grammar are correct. Step 2:

Step 3:
Go to www.google.com and sign in with your user name and password created in week one.
Step 4:
Go to "more" and choose "documents"
Step 5:
Choose "create new" and choose "form" from the list...a new screen will pop up, this is the form.
Step 6:
Name your form and provide a brief description about the form.
Example: Class: We spent the last three weeks covering the Industrial Revolution. This form was created so that I can check your understanding of the Industrial Revolution. Please answer each question.
Step 7:
Using the copy and paste method copy your first question from word and paste into the section "Question Title" section and then choose what type of question response you are requesting; multiple choice, text. If multiple choice add your answers seperately into each "answer option"
Step 8:
Repeat this process ten times and choose "save"
Step 10: Once you have completed two forms and pasted both URLS into the document above (due by 5/11) then look at 4 different forms on the document and intial in the box once you have completed it. Don't intial in your box, but in the box of the form you completed.

Type your full name, paste the URL and repeat this process to complete a second form.
